Position Description

Cottonwood Community Alternatives, Inc. is a private, non-profit agency that was established in 1991 to provide residential and day program services to Colorado citizens with intellectual and developmental disabilities (IDD). Cottonwood’s motto is “People Serving People,” and that is what we strive to do both for the individuals we support and our employees.

Cottonwood is seeking multiple energetic private duty nurses (RN or LPN) for a young, fun-loving, medically fragile individual living at home with his family.

SHIFTS:

  • Day Shifts: Saturday and Sunday 6am - 6pm
  • Overnight Shifts: Thursday, Friday, Saturday, Sunday 6pm - 6am
  • NOTE: Overnight shifts are awake overnight hours for skilled care. Nurses monitor O2 levels while the individual sleeps and provides hourly feeding via G-Tube. Other overnight tasks may included stocking supplies, bed rotation, medication administration, and other hands-on personal care as needed.

Flexible coverage can be coordinated with family and current nursing staff if a day/night is needed off.

ABOUT THE INDIVIDUAL:

  • This individual is an 19-year-old male who recently graduated high school and is looking forward to continuing his education at community college. He his very close with his family as well as the nurses who support him. Though he is unable to care for himself, he is of strong mind and heart and will win you over with his courage, humor, wit, positivity, kindness, and selfless actions. This young man loves his family, sports, church, Marvel, his friends, games, and music. Nurses working with this individual are expected to meet his socio-emotional needs as well as his medical needs. This includes engaging in meaningful conversation, playing board games and video games, assisting in communicating with friends virtually, and other means of enhancing his well-being. He requires frequent airway clearance/suctioning and line of sight care. He is able to verbally communicate his needs and wants, but needs hands-on assistance with all ADLs and recreational activities. Nurses supporting this individual can expect to be treated like family while on shift whether it's celebrating milestones together or participating in family game and movie nights!

JOB DUTIES include but are not limited to:

  • Airway clearance
  • Engaging in socio-emotional activities of the individual's choice and assist with his participation
  • Ordering/stocking medications and supplies as needed
  • Disinfecting and maintaining individual's room(s) and equipment
  • Monitoring respiratory status
  • Skilled nursing care for all hands-on personal care
  • Medication administration
  • Performing passive ROM as needed
  • Community outings with the individual and his family
  • Assisting in 2-person transfers

Must be familiar with and comfortable using the following DME and medical supplies:

  • Cough assist machine
  • G-tube and G-tube stoma care
  • Suctioning
  • BiPAP
  • CPT vest
  • Pulse ox monitor
  • Trilogy ventilator
  • Oxygen concentrator and other O2 supplies

REQUIREMENTS:

  • Active Registered Nurse or Licensed Practical Nurse
  • Valid Driver’s License
  • Clean Driving Record
  • Previous experience with DME listed above

  • Health Insurance (NOTE: Because the rate of pay and benefits are budget-based and determined by the individual's Medicaid waiver funds, this position is eligible for employee-only benefits. If employees wish to add family members to their plan, the rate of pay will be affected to factor that cost into the budget.)
